May 31st has passed and you completed your online health Assessment and biometric screen, congratulations, you earned $100 toward your rewards!
This July, Priority Health will call faculty and staff who presented opportunities for health improvement on their health assessment and biometric screen to engage with a health coach. faculty,staff, spouses and members of the household member program who engage with a coach who completed the Know Your Numbers components (Health Assessment and biometric screen) are eligible for an additional $100 into their Health Savings Account or paycheck.Faculty, staff, spouses and members of the household member program who presented no health risks from the health assessment and biometric screen will receive $100 for actively managing health risks without coaching. However if you would like to connect with a coach, please call the 1-800 number on the back of your insurance card.

Physician Documentation Form
For the Know Your Numbers component. If you had your height, weight, blood pressure, total cholesterol and HDL cholesterol tested between October 1, 2014 - May 31, 2015 you can have your provider complete this form in place of the on campus biometric screen. Form must be turned in to Priority Health by May 31st, 2015.
Women who are pregnant may substitute this form in place of the on campus biometric screen. Form must be turned in to Priority Healthy by May 31st.
Completion of the online health assessment is still required to be eligible for incentives.
