Make an Appointment
How to schedule an appointment:
To schedule an appointment you must call the GVSU Family Health Center at (616) 988-8774. You will need to provide basic information including your full name, date of birth, etc. We also ask about your insurance coverage.
With insurance information, we can let you know if your visit can be billed or not. However, we cannot guarantee coveraged due to variations in individual plans. If you are uninsured and have a low income, please ask our receptionist about our sliding fee scale.
If you are unable to call, you may also come to the GVSU Family Health center to schedule your appointment.
What to bring to your first appointment:
Bring your insurance card and any co-payment that is your responsibility to pay in accordance with your insurance policy. If you plan to apply for our sliding fee scale, please bring verification of income such as pay stubs, social security or disability payment letter, W-2 forms, or a letter from your employer, and a $15 copay. For any self-pay patients that will not be applying for the sliding scale, there is a minimum charge of $50 per appointment that must be paid in full prior to being seen.
