Important Information About Your 2025 Form 1095-C

Inactive Employees
In accordance with IRS guidance, Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 calendar year is available upon request. Previous GVSU employees may obtain a copy of their Form 1095-C at no charge by submitting a request via email: [email protected].

When submitting a request, please include your full name, current mailing address, and the last four digits of your Social Security number so we can process your request accurately.

Active Employees
Your Form 1095-C is now available in Workday for your review and printing. To access your form, login to Workday and navigate to your profile. From there, select Actions > Benefits > View My ACA Forms. You will be able to view and print your Form 1095-C directly from this section.
