Active Management Definition
Members -Risks Identified
Members who have identified risk areas from their physical or Clinical Health Risk Assessment must engage in management of their condition by working with a health coach offered by UMR’s Wellness Program. For example, participating in disease management if you have been identified in a health risk area such as smoking or weight management.
Members-No Risks Identified
If no risk factors appear from your screening “active management” means following your preventive care schedule according to your age and gender.

How will my active management be tracked?
UMR will manage and track individuals who are actively participating in disease management and will be able to identify individuals who are following their preventive care for the year.
In 2013 if active management is taking place, and you participated in Healthy Choices ie: KYN and 2 wellness activities per quarter, you will receive your incentive in 2014.
