Know Your Numbers

KNOW YOUR NUMBERS (Receive $100 for completing the following) :

  1. Sign up for your on site appointment with Wellness Inc.
    OR complete the biometric components with your own health care professional
  2. Complete your Health Risk Assessment (HRA) online March 2-23, 2012 or October 1-31, 2012.

All benefit eligible participants who complete the Know Your Numbers components will receive $100 into their HSA in the first quarter of 2013! Those who are not participating in the High Deductible Plan will receive the reward in their paycheck*.

HOW DO I CHECK TO MAKE SURE MY HRA IS COMPLETE?
1. Click on the link above that applies to you (UMR member or non member)
2. On item 8, under Health Risk Assessment, there will be the following statement:  "Congratulations! You have completed your Health Risk Assesssment for the current wellness cycle".
