Disability Statement: This form is used to affirm that you are applying for federal loans and that the loans cannot be discharged in future based on a current disability.
Concurrent Enrollment Forms: These forms are for students who are enrolled in an approved community college and Grand Valley concurrently and would like to have their enrollment from both schools used to determine aid eligibility at Grand Valley. As a reminder, students who are dual enrolled at two institutions will not be eligible to receive aid at Grand Valley and another institution during the same period.

Dependency Status Appeal: This form is used to consider a dependent student independent for financial aid purposes.
Documentation of Children and/or Legal Dependents: This form is to document that a student who reports having a child or legal dependent has sufficient income to support the child or legal dependent. It is also used to document that the student provides more than 50% of the support for the child or legal dependent.
Household Size/Number in College Statement: This form is used to verify the names and ages of the members in your household.
Parent PLUS Loan: This form is used to apply for the Parent PLUS Loan.
Release of Information Form: This form is used to release financial aid information to a third party.
SAP Appeal (Satisfactory Academic Progress)-Undergraduate Students: This form is used to appeal financial aid status.
SAP Appeal (Satisfactory Academic Progress)-Graduate/Doctoral Students: This form is used to appeal financial aid status.
Scholarship Appeal Form: This form is used to appeal the loss of a scholarship.
Scholarship Reinstatement Form: This form is used to request reinstatement of scholarship once you have met the renewal criteria.
Proof of US Selective Service Certification: This form is used to confirm your registration status with the Selective Service.
Special Circumstance Forms for Dependent Student: These forms are used to request a reconsideration of aid based on a change in your or your families financial situation. Contact the Financial Aid Office to discuss your situation for the current academic year 2013-2014.
Special Circumstance Forms for Independent Student: This form is used to request a reconsideration of aid based on a change in your or your families financial situation. Contact the Financial Aid Office to discuss your situation 2013-2014.
Spring/Summer Supplemental Financial Aid Application: This form is used to request a review of financial aid eligibility for the spring/summer semester.
