L3 Account Request Form

Students that graduated in April 2024 or earlier for funds from their L3 account.  Eligibility and amount information can be found at the L3 Account website.  Please complete the following form to have your eligibility reviewed.  If you qualify, funds will automatically be added to your account. If you have already received your full $1000 lifetime award, you will not be awarded any additional funds.  Please contact us if you have any questions at [email protected].
