Student registered for a Multiple Delivery (MD) course along with another online course, must submit this form to confirm that they will attend the MD course fully in-person for the duration of the semester.

Multiple Delivery (MD) courses are courses where a student may elect to attend in person or synchronously online. In order for an international student to meet the full-time enrollment requirements of their visa status when taking multiple delivery course(s), the student must regularly attend the course(s) in person over the course of the entire semester.
