Application for Re-Entry: Students who are in good standing but have not been enrolled in a course for two or more semesters must submit this form to the Registrar's Office.

Application for Re-Admission after Dismissal: Students who have been asked to leave for academic and/or disciplinary reasons must file an appeal to be re-admitted to the university.  All materials need to be submitted at least 10 business days in advance to the start of the semester.
