Fall 2024 Name Badge Order
MPH Students - order your official program name tag!
Current and incoming MPH students have until noon, August 24 to order their official badge. All Cohort 13 students and anyone needing a replacement should order at least one name tag at this time! Some students may find it beneficial to order additional name tags for convenience or to serve as a backup should one get lost or damaged. The name tag portion of the MPH dress code will be enforced once the badges from this order have been distributed.
Price per badge is $7.00.
Deadline to order AND make payment is August 24 (12:00 p.m./noon)
Payment can be made with a credit card through the form, or you may indicate on the form that a check payment will be submitted to the Public Health office. Check payments delivered in person or via the mail must be received by our office no later than 12:00 p.m. on August 24.
The MPH bulk order only happens once per year, so we encourage you to get your name tag(s) now. Badges ordered outside the bulk order window will cost significantly more.
