If you are returning to Grand Valley or are new to the University, you are considered a prospective student and must complete this form to schedule an appointment.
Our Office will Call You
Once you have submitted your form, a College of Health Professions receptionists will give you a call within 24 hours during normal business hours to schedule your appointment.
Types of appointments we offer
In-Person
Meet with an advisor face to face! We meet with students on both the Health Campus and Allendale Campus
If you are sick on the day of your appointment we ask that you either switch to a phone/zoom appointment or call our office to cancel and reschedule.
Virtual
Meet with an Advisor via Zoom! Prior to your appointment you will receive a Zoom link for your scheduled appointment.
It is important that you have reliable internet service for a minimum of 30 minutes.
Phone
Chat with an advisor over the phone! On the day of your appointment the advisor will call you at the phone number provided in the appointment registration process.
This format is ideal for quick questions!
