Students and employees are required to be vaccinated against COVID-19 unless approved for an exemption or postponement. The Exemption/Postponement Form can on found online.
Vaccines are available now from health care providers, pharmacies, and local health departments. Coronavirus - COVID-19 Vaccine (michigan.gov)
Faculty, staff, and students are required to record their vaccination status on their online self-assessment. Please do so at your earliest opportunity. If you lost your COVID-19 vaccination record, the Michigan Department of Health and Human Services (MDHHS) has launched the Michigan Immunization Portal. Michiganders ages 18 and older with immunization records posted in the Michigan Care Improvement Registry (MCIR) will be able to locate their own record online and download, save or print this information. Users must create a MILogin account at Michigan.gov/MiImmsportal and upload a valid government issued photo ID, such as a driver's license, state ID or passport. There is no cost to access the portal.
HOW TO GET A COVID-19 VACCINE
GVSU is holding two COVID-19 Pfizer vaccine clinics for anyone who needs a first or second dose, and for those who qualify for a booster dose.

Booster Doses
The FDA, CDC, and State of Michigan have authorized booster doses of the Pfizer COVID-19 vaccine. The Pfizer booster is authorized for individuals who have been fully vaccinated with the Pfizer COVID-19 vaccine for six months or longer and belong to one of these three eligible groups:
- Anyone ages 65 and older.
- Anyone ages 18 and older with a medical condition that increases their risk of severe COVID-19 illness, such as obesity, diabetes, high blood pressure, kidney disease, and others.
- Anyone ages 18 and older who lives or works in a high-risk setting for being exposed to COVID-19, such as residents of congregate living facilities, health care workers, first responders, educators, childcare staff, grocery store workers, and others.
Remember to bring your vaccination card to the vaccine clinic.
Moderately to severely immunocompromised persons can get another dose of either the Moderna or Pfizer vaccine after 28 days has passed since their second dose. However, only the Pfizer vaccine will be provided at our on-campus clinics. Those who qualify for a booster dose of Moderna should contact their health care provider or can find other locations on vaccines.gov.
