Resume/Essay Upload

Please upload one document that includes your up-to-date resume and your 3 field of practice essays. This document will be sent to the prospective field placement site.  Please place your resume as the first page of your document.  Ensure your resume includes your name!  The proceeding pages should be your three one-page essays, one for each of the three fields of practice you wish to be considered for your Field Education. The essays will serve as a sample of your writing for the person who will interview you at the field site, therefore they should reflect your best written work. In each essay please address the following:

  1. What do social workers do in this field of practice?  Who do they work with?  What settings might they work in?  Answer these questions by identifying the various roles and positions a Bachelor level social worker might hold within this field of practice, such as engaging with individuals, families, groups, organizations and/or communities.
  2. Share your interest and suitability for this field of practice.  Why do you want an internship in this field of practice? What makes you a suitable candidate for an internship in this field of practice?

Each essay must be typed and no more than 1.5 pages in length.  Use course texts, reputable web sites, and/or journal articles to support your research. Appropriately cite sources where necessary.  A cover page is not needed, but please label the field of practice at the top of the essay.

Personal and Professional Information

Criminal History and Background Information

Most Human Service Agencies require student interns to submit to a criminal history background check. It is therefore critical that you are informed of how a criminal history background may impact your Field Education. Please carefully read the statements below.

  • I understand that I am required to disclose any past, current, or pending felonies, criminal charges, convictions, listing on the central registry, and/or penalties in this application.
  • I understand that failure to disclose any past, current, or pending felonies, criminal charges, convictions, listing on the central registry, and/ or penalties in this application may result in disciplinary action by Grand Valley State University School of Social Work.
  • I understand that along with the rest of my Field Education Application, the “Criminal History Background Information” portion of my Field Education Application, will be made available to Field Sites that I will interview with for Field Education Placement.
  • I understand that even though the “Criminal History Background Information” portion of my Field Education Application will be made available to Field Sites, I am required to disclose all past, current, or pending felonies, criminal charges, convictions, listing on the central registry, and/or penalties when I interview for Field Education Placement with a Field Site.
  • I understand that failure to disclose any past, current or pending felonies, criminal charges, convictions, listing on the central registry, and/or penalties when I interview for Field Education Placement with a Field Site will result in disciplinary action by Grand Valley State University School of Social Work.
